Benefit Plans Overview
The biggest advantage is that employers with 2–50 employees gain access to fully insured, underwritten large-group products with composite rates, offering a cost-effective alternative to overpriced ACA plans. For groups with 51+ employees, spreading risk across a larger pool can help stabilize renewals and provide long-term cost predictability.
Eligibility & Participation
In order to qualify for the EmployerPulse Association Health Plan (AHP), the following criteria must all be met:
1. Your company must have 2-99 eligible (ATNE) employees.
2. Your company’s SIC code must be considered manufacturing, or wholesale and retail. Please click on the appropriate link for verification.
3. Your company must be domiciled in one of the following Wisconsin counties.

- Buffalo
- Columbia
- Crawford
- Dane
- Grant
- Green
- Iowa
- Jackson
- Juneau
- La Crosse
- Lafayette
- Marquette
- Monroe
- Richland
- Sauk
- Trempealeau
- Vernon
- Wood

Obtain an Underwritten Quote
Less Than 10 Employees
If your group has fewer than 10 employees, is a virgin group, or has low participation, you must complete the following forms:
More Than 10 Employees
If your group has more than 10 employees, you must complete the following forms:
Census Form for a Manufacturer Group or Census Form for a Wholesale/Retail Group
Provide a tax id number within the “notes” sections of the quote form
Once the information is fully completed, please email it to rbaker@wmc.org. Please note that this process can take 7-10 days for processing.
Network & Provider Search
Please refer to your UHC quote for what network to search. The most popular options are Choice and Choice Plus Network.
Enrollment Process
Manufacturing Group Installation
Attached are the required documents to enroll your group in the WMC EmployerPulse Association Health Plan.
Once completed, please return all forms to rbaker@wmc.org and vo_nationalaccounts@uhc.com. To ensure a smooth enrollment process, please submit the installation paperwork at least 2-3 weeks prior to the requested effective date.
Please note that incomplete forms may cause delays in processing, so be sure all documents are fully completed before submission. If you have any questions or need assistance, feel free to reach out.

Wholesale/Retail Group Installation
Attached are the required documents to enroll your group in the WMC EmployerPulse Association Health Plan.
